FOREST HILLS COUNTRY CLUB
Entries are open to junior golfers (male and female) who: (a) will reach the age of 12 by July 12, 2010 and (b) will not reach the age of 18 by July 12, 2010 and are affiliated with or a member of a club which is a Member Club of the Metropolitan Amateur Golf Association. The entrant must have an up-to-date and current USGA GHIN Handicap Index. This entry and USGA GHIN Handicap must be certified by an Association (or Club authorized to issue a USGA GHIN Handicap). Entries are subject to rejection at any time (including during the Championship) by the Championship Committee. The reason for rejection may include unbecoming conduct. Field will be 120 applicants with the lowest USGA GHIN Handicap Indexes.
Competition will be 36 holes (gross), at stroke play, flighted by age groups (in Boys and Girls divisions) as follows: 12-13 years old; 14-15 years old; and 16-17 years old. All Girls will play from one tee; Boys 14-15 & 16-17 will play from same tee.
MONDAY, JULY 12 - First Round, 18 Holes
TUESDAY, JULY 13 - Second Round, 18 Holes
Lowest gross scorer for 36 Holes will be Champion. Any tie(s) for the overall champion will be decided immediately by a sudden-death hole-by-hole play-off. For all other ties within a specific age group, see Appendix I of the USGA Rules of Golf, How to Decide Ties, Paragraph (c).

Overall Champion: James Holtgrieve or Barbara Berkmeyer Trophy for a period of one (1) year; Gold Medal
Runner-Up: Silver Medal
1st Place in each age group: Gift
Runner-Up in each age group: Gift
NO OTHER REMUNERATION: By this application, an entrant certifies that he/she is not entitled to remuneration of any kind for participation in this Championship other than the aforementioned prizes.
FOOTWEAR
Forest Hills Country Club is a spikeless facility. Shoes with traditionally designed spikes, comprised either partially or entirely of metal, are prohibited. Penalty for breach of this Condition:
DISQUALIFICATION
ENTRIES: Form, Fee, Closing Date
All players shall apply on the attached form. Copies are acceptable.
$60.00 entry fee shall be paid by every applicant. Entrant may file only one (1) entry. Entry fee will be refunded regardless of the reason if the Metropolitan Office is notified prior to the close of entries June 23, 2010. After the close of entries, entry fee will be refunded only after written proof to the Championship Committee of injury or illness and such written request for refund is made prior to July 7, 2010. No refunds for any reason if written request is made on or after July 7, 2010.
Entry Application and fees must reach the Association’s Office at the address listed on the entry form by 5:00 p.m., June 23, 2010. (Entries by telephone, telegraph or fax are not acceptable.)
REGISTRATION AT FOREST HILLS CC
All players must be ready to play at their appropriate starting point and published starting time.
PRACTICE PRIVILEGES
Practice privileges (one practice round) at Forest Hills CC are extended to players in the Championship after the close of entries, at a time not disruptive to their member play. Information on practice privileges will be sent to players after the close of entries via email address provide on this entry.
CADDIES; AUTOMOTIVE TRANSPORTATION
A player may provide his own caddie at any time during the Championship, provided the caddie is not a participant in the Championship, or a player may carry his own clubs. Players and/or caddies may not use automotive transportation during the Championship Proper. Players must walk at all times during a stipulated round.
DISTANCE-MEASURING DEVICES
The Local Rule as prescribed in Appendix I of the USGA Rules of Golf shall be in effect. A player may obtain distance information by using a device that measures distance only. If a player uses a distance-measuring device that is designed to gauge or measure other conditions (e.g., gradient, wind-speed, temperature, etc.), the player is in breach of Rule 14-3, for which the penalty is DISQUALIFICATION, regardless of whether any such additional function is actually used.
RULES OF GOLF
Play will be governed by the United States Golf Association Rules of Golf. Questions shall be settled by the Championship Committee whose decision shall be final.
TIME OF STARTING
The Optional Condition for Time of Starting, as set forth in Appendix I of the USGA Rules of Golf, shall be in effect.
CONDITIONS, SCHEDULES
The Championship Committee reserves the right to alter any of the conditions and schedules herein. The decision of the Championship Committee in any matter shall be final.
Only brands of golf balls on the most current USGA List of Conforming Balls may be used. (List is available at www.usga.org)
The Optional Condition, as set forth in Appendix I of the USGA Rules of Golf, which places a limitation on golf balls used during a round (One-Ball Rule), shall be in effect.
Only Driver Heads on the most current USGA List of Conforming Driver Heads may be used. (List is available at www.usga.org)
PERSONAL APPEARANCE
During all phases of the Championship, Competitors must be neat in appearance with respect to clothing and personal grooming. The Championship Committee shall decide whether this requirement has been met and has authority to withdraw the entry of a Competitor who does not comply. The wearing of Bermuda length shorts is permitted.
NOTICE CONCERNING AMATEUR STATUS
Participation in this event is conditioned on Amateur Status. The USGA Rules of Amateur Status shall apply. The Championship Committee reserves the right to disqualify any participant for not having Amateur Status. Disqualifying events include (i) competing for cash prizes in any amount and (ii) competing for merchandise prizes exceeding $750 in retail value.
